Requesting the annual 1095-C Form
Alternative manner of furnishing statements.
The IRS no longer requires employers to automatically mail Form 1095-C to health plan participants enrolled in the CPS Energy Health Plan. In previous years, CPS Energy mailed this form to all participants in January. Beginning in 2026, employees and retirees may request a copy of their Form by contacting Employee Benefits at [email protected], at 210-353-2900, ext. 1, or through the link Request your 1095- C form. The form will be mailed or emailed no later than 30 days after the date of the request. As a reminder, this form is for your records only and is not required to file your annual income tax return.
